To CompleteYour Enrollment;

All first time group lesson students (even for those with a Free Lesson Certificate), after we receive this form please call Norri or Melanie at Guitar Showcase (408)-377-5864 with your credit card information which is required as a deposit ($85.00). This is strictly a deposit and will only be charged if you fail to attend your assigned lessons. Remember, there are other students waiting for your spot. Don't waste this opportunity.
